I came across this helpful guidance from forusall demystifying the 401k plan adoption agreement . https://www.forusall.com/401k-blog/401k-adoption-agreement/ Two important additional points are: 1. you need to answer the question whether the plan sponsor belongs to a controlled group correctly (your plan may not qualify if you don’t); and 2. new adoption agreements to reflect changes in the law are required to be signed on a schedule established by the IRS. This is not a one time job.
